Please enter your Hospital name in the 'Organisation' field, the street name in the 'Address' field, district or county in the 'State' field, postcode in the 'Zip' field and your Department name in the Additional Information area.

Please take care to enter your email address accurately as this will be the only means by which we contact you about your registration.

Please do not use all lower case or all upper case as these entries then have to be manually amended prior to printing.

The registration fee of £75 shown here is adjusted according to the radio buttons you select. The final fee after discounts and additions is displayed on the next page. The ACB member (not trainee) 'Yes' button deducts £15; the ACB/IBMS trainee 'Yes' button deducts £25. Please do not click 'Yes' to both the ACB buttons as this will generate an incorrect fee.

After submission, you should see a successful registration page and then receive an email acknowledgement to the email address you have entered with instructions for payment. If this does not arrive within 24 hours, please contact Jonathan Middle immediately. (We are aware that some emails do not get through Trust firewalls.)
